Where is UHealth Sports Medicine located?

How can I make an appointment at specific location?

All appointments are made by calling 305-243-3000. Request a specific location for your appointment while speaking with the operator.

Where is University of Miami Hospital? Is it on the campus of the University of Miami?

The University of Miami Hospital is located on the medical campus of the University of Miami, adjacent to Jackson Memorial Hospital and near downtown Miami. It is approximately 9 miles northeast of the University of Miami’s Coral Gables campus.

Where do I park when I arrive for my appointment?

How do I let my doctor know how my experience has been?

What is the difference between a primary care sports medicine physician and an orthopedic surgeon?

Primary care sports medicine physicians specialize in the non-operative treatment of musculoskeletal conditions. Orthopaedic surgeons, on the other hand, are also trained in the operative treatment of these conditions. UHealth Sports Medicine has both primary care sports medicine physicians and orthopedic surgeons.

Can I see your primary care sports medicine physican as my primary care physician?

No, UHealth Sports Medicine physicians only see patients for musculoskeletal concerns that affect the ability to participate in sports, exercise or simply maintain an active lifestyle.

I am a referring physician with a patient to refer to UHealth Sports Medicine. How can I contact a provider?

My physical therapy prescription has expired. How can I renew it?

You or your physical therapist may request a renewal to your physical therapy prescription by emailing us at uhealthsportsmedicine@med.miami.edu or by calling 305-689-5500.

What do I bring to my appointment?

Bring your health insurance card, a photo ID, and any previous images or medical records pertaining to your present issue. If you have previous MRIs, xrays, etc., bring the actual CD or hard copy films to the appointment, not just the radiology report. If you want to complete your new patient forms in advance, view and print them here to bring with you: English or Spanish.

I’m scheduled for surgery. How do I know what time my surgery will be?

A representative from surgical scheduling will call you after 3:00pm the day before your scheduled surgery date to notify you of when you need to arrive. If your surgery is on a Monday, they will call on the Friday prior.

I am having surgery. What do I need to do before my operation?

For surgery, where do I go on the day of my operation?

On the day of your surgery, report to the first floor Ambulatory Surgery Center in the south building of the University of Miami Hospital, 1295 NW 14th St, Miami, FL 33125.

Who do I contact with questions about billing?

You may receive separate bills from the doctor or provider and from the University of Miami Hospital, where our clinics are based. For provider billing inquiries, call 305-243-2900. For University of Miami Hospital billing inquiries, call 866-506-6438.
