Primary Care Sports Medicine Fellowship Application Process

Positions

There is one position available each year in the Primary Care Sports Medicine Fellowship.

How to Apply

You must apply through the Electronic Residency Application Service (ERAS). This can be done through your medical school dean’s office.
The application form and all supporting documentation must be submitted by October 15th of the year preceding the appointment date. The academic year begins in July.

Materials Required to Apply

To apply to the program, visit the ERAS website. All applications to Jackson Memorial Hospital/University of Miami Hospital Primary Care Sports Medicine Fellowship must include these application materials.

  • Curriculum vitae: describe your activities with professional societies, publications, and any other relevant information about your education or experience.
  • Personal statement of professional goals.
  • Copy of graduate school and or medical school diploma.
  • Dean’s letter: ask your medical school to send a letter directly to the education coordinator listed in the application process of the program to which you are applying.
  • Official final transcripts: transcripts must indicate degree earned and date conferred from each graduate school and medical school you attended. If you receive an appointment, official copies will be requested. Provide certified or notarized English translations of documents originally written in a language other than English.
  • Official test transcripts for all applicable examinations: USMLE, LMCC, COMLEX, NBOME, FMGEMS, FLEX, or NBME.
  • Valid ECFMG certificate required if you graduated from a medical school outside the U.S. or Canada.
  • Three original letters of recommendation, or if specified in your program description, letters from physicians or faculty members who are thoroughly familiar with your work. If you have had prior residency training, ask your program director for a letter of recommendation. Each letter should include the writer’s opinion of your professional, academic, and personal qualifications, as well as an opinion of your potential success in the field for which you are applying. Postdoctoral fellowship applications should have one of these letters from your thesis adviser.
  • Copy of completion certificate from each of your prior residency and/or fellowship training programs.

In addition to what is listed on the application materials website, this fellowship also requires:

  • A letter from your program director, which can be uploaded in ERAS as the fourth letter of recommendation.
  • USMLE or COMLEX scores, which also can be uploaded in ERAS.

All appointments are made through the National Resident Matching Program (NRMP). Candidates must register through the NRMP website.

Applicants considered for an appointment will be invited to visit Jackson Memorial Hospital/University of Miami Hospital Primary Care Sports Medicine Fellowship for a personal interview with the program director and selected faculty. Interviews are conducted between October and December each year.
